Understanding the psychological basis of obesity PI The obesity epidemic is one of the largest public health crises of our time. Compared to normal weight individuals, individuals living with obesity experience a dramatically reduced quality of life, are at increased risk of ill health and die younger. Obesity has been traditionally characterised using an ‘organic’ or biological model of disease: obesity is persistent because homeostatic systems defend against weight loss and the reason people with obesity die young is attributable to the biological strain of excess body fat. Here we will test an alternative ‘psychological burden’ model of obesity. We propose that the widespread stigma attached to heavier body weight produces a psychological burden that is instrumental to understanding the persistence, progression and health consequences of obesity. We will first make use of longitudinal cohort studies to quantify the role that psychological factors collectively have in explaining (i) whether a person remains overweight or obese vs. returns to a healthier weight and ii) the development of severe obesity. Next, we will exploit data from large-scale prospective studies to examine how much of the obesity-related physical disease burden (impaired health, cognitive decline, early mortality) is explained by the psychological burden of heavier body weight. By adopting this approach, we will also be able to identify potentially modifiable psychological factors that protect against the development of obesity and the progression of obesity-related disease. Finally, we will collect new data to understand the key underlying processes that contribute to the psychological burden of obesity. Our proposed psychological burden model has potential to change the way that the medical and public health community understand the development of obesity and improve treatment approaches to reduce obesity and the obesity-related disease burden. 01/03/2022 28/02/2024 8036
